JOB HIGHLIGHTS
Responsibilities:
- Provide case management services and maintain a caseload of Long-Term Support Clients supporting them in a variety of job sites
- Provide direct skill development/job coaching support for individuals at their community job sites
- Possess awareness of realistic work expectations regarding quality of work and appropriate behavior
- Ability to perform personal cares in a discreet, respectful manner (i.e.: toileting, eating, mobility, behaviors)
- Ability to effectively communicate to act as a liaison between employers and employment services staff as well as individuals care team (funding sources, employers, families/caregivers)
- Ability to work independently and creatively problem solve as needs arise for individuals
- This position requires a company email address which requires an application to be downloaded on your personal cell phone for multi-factor authentication (MFA) to ensure cyber security.
- Reliable vehicle (unless determined exempt with HR)

- Typical schedule is Monday-Friday

Requirements:Qualifications:
- High School graduate or equivalent
- Post-secondary training in human services or related field/experience working with people with disabilities may be substituted
- Two (2) years of work experience
- Excellent communication skills (verbal and written)
