As a Designated Coordinator you will support our Day Support Services by serving as an agency point of contact for consumer teams, attending consumer meetings, and ensuring compliance with 245D documentation.

Hours and Benefits of the Designated Coordinator:
- The starting salary is $42,000-$50,000 per year based on previous experience and education.
- Typical hours are Monday to Friday from 8:00am-4:30pm
- Located in Rochester, MN

Basic Responsibilities of the Designated Coordinator:
- Serve as a primary point of contact for consumer teams, county case workers, and DHS representatives.
- Assist in the creation and implementation of consumer support plans.
- Schedule and coordinate consumer meetings.
- Prepare necessary paperwork for consumer meetings.
- Attend and lead consumer team meetings.
- Work closely with direct support staff to ensure positive person-centered outcomes
- Complete administrative tasks in a timely manner.
- Perform any additional duties as assigned.
- Attend all required and requested staff meetings and training.
- This role is primarily administrative in nature, but will occasionally provide hands-on support in our Day Services program.
Salary : $42,000 - $50,000
