Position Overview: As a Skilled Nurse Case Manager, you will be a key player in our patient care team, managing and coordinating comprehensive care plans for patients in their homes. This role offers flexibility, typically working Monday through Friday, 8:00 am to 4:30 pm, with minimal to no nights, weekends, or holidays.
Key Responsibilities:
- Develop, implement, and manage individualized care plans.
- Conduct patient assessments and monitor progress.
- Coordinate care with interdisciplinary teams including physicians and therapists.
- Educate patients and their caregivers and support team about health conditions and care plans.
- Ensure compliance with all regulatory standards and company policies.
- Maintain accurate and thorough documentation.
Qualifications:
- Valid RN license in the state of Iowa.
- Reliable transportation and a valid driver’s license.
- Strong communication and organizational skills.
- Ability to work both independently and collaboratively.
- A passion for providing home care and supporting mental/behavioral health needs.
