Optimae LifeServices, Inc. in Adel is hiring for a full-time Residential and Community Service Coordinator to work in Dallas County. Applicants should be caring, creative, and passionate about working with people experiencing Mental Illness, Intellectual, and/or Developmental Disabilities. The right candidate will join our leadership team in providing high quality services to our customers and leadership to our Team Leaders and Direct Support Professionals in a team based environment.
Qualifications:
- Be at least 18 years of age
- A high school diploma or GED is required.
- Bachelor’s Degree from an accredited university, preferred
- Two (2) years post high school education in a human service related field or two (2) years of full-time equivalent experience serving persons with disabilities is preferred.
- Be able to pass a required background check which includes Child and Dependent Adult Abuse, criminal record, and driving record checks
Duties:
- Coordinate and lead service delivery and service planning process to ensure the provision of service based on customer goals and desires
- Coordinate and lead staff coaching using pro-active problem solving, team based methods for customer-driven services, and monthly staff plans for development.
- Maintain comprehensive customer records in compliance with DIA, DHS, CARF, and other applicable standards.
- Coordinate services and procedures to provide for the health and safety of customers.
- Maintain human and civil rights in a manner consistent with Optimae LifeServices’ Mission, Vision, and Core Values.
- Coordinate all transfers and discharges with customers including discharge planning, future services, and review.
- Work alongside administration to market Optimae LifeServices’ services, collect data and maintain management information system, design, measure, and implement needed improvements and recognize accomplishments.
- Provide direct services when needed to meet the customers health and safety needs.
- Provide on-call support by carrying a cellphone when service needs dictate. Respond to calls in a timely manner and be available to provide direct support to customers when needed.
- Work as a team member to achieve financial objectives.
- Interview, hire, discipline, and terminate employees as needed.
- Perform other tasks as needed to support our Leadership Team.
