What are the responsibilities and job description for the Community Liaison position at Optimal Health Services Inc?
JOB SUMMARY: The Community Liaison is responsible for establishing and maintaining positive relationships with customers and referral sources within the Community. The Community Liaison will round in all facilities the day of or day after admission to ensure the patient and community have required paperwork, chart/s are set up and in compliance per the facility requirements, DME, medications and supplies are in place. The Community Liaison will build and maintain customer service by anticipating and responding to facility and internal customer needs.
ESSENTIAL JOB FUNCTIONS:
- Ensure the educational needs of the community related to hospice care and services are assessed and met on an ongoing basis.
- Continuously conduct audit and review of charts to ensure compliance.
- Establish and monitor community, customer, and patient perceptions of Bristol Hospice as a high-quality provider of hospice services in the community.
- Maintain comprehensive working knowledge in the field of community needs. Receive hospice referrals. Conference with Clinical Supervisor and/or Director of Patient Care Services to collaborate and schedule intake activity.
- Follow intake referral processes.
- Serve as a liaison between facility, physicians, hospitals, skilled nursing facilities, and community and hospice program with coordination of referrals/admission process and services.
- Promptly respond to customer and referral source requests and concerns.
- Complete consent forms as assigned and indicated.
- Submit associated documentation in accordance with administrative processes.
- Provide patient/family education related to hospice philosophy and care.
- Comply with accepted professional standards of practice and principles.
REQUIRED EDUCATION & EXPERIENCE:
- GED or High School education preferred
- One to two years of experience in Hospice Sales preferred
REQUIRED KNOWLEDGE, SKILLS & ABILITIES:
- Must have excellent observation, verbal and written communication skills, problem solving skills.
- Must be able to perform prolonged or considerable walking or standing
- Must be able to lift supplies and equipment
- Must be able to perform considerable reaching, stooping, bending, kneeling, and/or crouching
- Must have visual acuity and hearing.
OTHER REQUIREMENTS:
- Must be flexible in work hours
- Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software program
WORK ENVIRONMENT:
- Must comply with company policies, procedures, and processes in addition to Hospice rules and regulations
- Must be dependable and flexible in work schedule

- Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
- Must be willing to submit to a criminal background check as well as a drug/alcohol screening upon hire and periodically thereafter
PHYSICAL ENVIRONMENT:
- Must demonstrate vital sensory abilities including near vision, far vision, perceptual speed, visual color discrimination, depth perception and hearing sensitivity
- Must have finger dexterity, arm-hand steadiness, and manual dexterity
- Must be able to communicate with patients, families, and other health care workers
- Speech clarity, speech recognition and written expression are vital abilities for performing the job.
- Will be required to lift heavy items on a frequent basis. Must be able to lift and carry up to 50 lbs. with or without assistance and must be able to spend a prolonged amount of time standing, walking, bending, and stretching
