Hospice Community Liaison's Responsibilities:
- Develop and maintain relationships with healthcare professionals, including physicians, discharge planners, case managers, and social workers, to promote awareness of Optimal Hospice services.
- Educate healthcare providers and community partners about the benefits of hospice care and palliative services.
- Collaborate with internal teams to facilitate timely admissions and transitions of care for patients referred to hospice services.
- Conduct outreach activities, including presentations, in-services, and networking events, to raise awareness of Optimal Hospice within the community.
- Act as a resource for patients, families, and community partners seeking information about hospice services and end-of-life care options.
- Represent Optimal Hospice at community events, health fairs, and professional meetings to enhance our visibility and reputation in the community.
- Maintain accurate records of outreach activities, referrals, and communications in accordance with company policies and regulatory requirements.
Pay: $75,000-90,000

Qualifications:
- Bachelor's degree in healthcare administration, marketing, business, or related field preferred.
- Minimum of 1 year of experience in healthcare sales, marketing, or community relations, preferably in hospice or healthcare services.
- Strong interpersonal and communication skills, with the ability to establish and maintain relationships with diverse stakeholders.
- Knowledge of hospice regulations, Medicare/Medicaid guidelines, and healthcare industry trends is desirable.
- Self-motivated and results-oriented, with the ability to work independently and as part of a team.
- Proficiency in Microsoft Office suite and customer relationship management (CRM) software is preferred.
- Valid driver's license and reliable transportation required.
