Overall Function:
The In-Home Outreach Counselor (IHOC) is responsible for providing Family Preservation Intervention Services which could include: Alternative Response Services (ARS), Family Preservation / Family Reunification (FP/FR) to eligible families. IHOCS participate in the Multidisciplinary Case Planning Committee (MCPC) meetings, conduct IHOC visits, provide supplemental services and provide linkage services.
ESSENTIAL JOB DUTIES:
- Develop / complete the initial services plan as well as making updates to the plan as needed.
- Must visit all family members on the service plan for at least 1 hour weekly for IHOC visits; four visits to be conducted once a week four times a month.
- Schedule Multidisciplinary Case Planning Committee meetings.
- Produce and submit monthly reports to DCFS.
- Execute and monitor the service plan.
- Provide case management, crisis intervention, and advocacy for the family during IHOC sessions as needed or required.
- Link the family as needed to additional supports or services.
- Attend and participate in mandatory training and in-services as required.
- Performs other tasks and responsibilities as deemed appropriate by Clinical Supervisor.
- Must be available to work some evening and weekends, as needed.
- Other duties as assigned.
Qualifications
- Master’s degree in social work, Psychology, Marriage and Family Counseling, or related field required or A bachelor’s degree in behavioral sciences or a related field in social services and experience providing direct case management services in a social service agency, with a County approved educational waiver.
- At least 3 years working foster youth or foster families is preferred.
- Bilingual in English/Spanish preferred.
- Training and experience in Evidence-based Practices (EBP’s) preferred.
- Computer literacy.
- Knowledge and/or competence in LA County DCFS Family Preservation policies and/or procedures preferred.
- Excellent organizational skills and attention to detail required.
- Ability to manage time effectively and be flexible with hours and duties.
- Valid California driver’s license; current automobile insurance; have and maintain a clean driving record acceptable to the organization’s insurance company
- Must be able to be pass a fingerprint background investigation.
- Must be able to pass a TB test, Physical Exam, and Drug Screening.
- All applicants MUST be fully vaccinated.
- All employees, regardless of position service as role models for children and adolescents who are serviced by OYHFS programs. Therefore, each employee must at all times be emotionally stable and able to function effectively with children, adolescents and adults who may have mental or behavioral problems. The staff must be able to demonstrate appropriate daily behavior, appropriate expression of emotions, as well as appropriate role modeling. Hostility, aggression and unnecessary or inappropriate physical actions as well as inappropriate emotional expression are not acceptable.
In accordance with CA law, the expected pay range for this CA position is posted with this position. The actual compensation may be determined based on bona fide factors such as education, experience, and other factors permitted by law.
Salary range for this position: $30.00 / hr - $41.53 / hr (annually)
Hiring Range for this position: $30.00 / hr - $30.00 / hr (annually)
