We are currenlty looking for a fulltime RN to join our team based model of care delivered in a kind and compassionate manne r. This position is 100% grant funded . The RN Care Coordinator ( RNCC) works with an APRN, Medical Assistant, and Case Manager supports the clinical management of incarcerated patients transitioning to be released by providing health needs assessments within the correctional facilities, develops patient engagement, and facilitating video encounters with the APRN. The RNCC wil support the development of care plans to reduce care fragmentation and improve outcomes. The RNCC also provides nursing care within the Nurse Practice Act framework, requiring clinical expertise, communication skills, problem-solving abilities, adn knowledge of healthcare systems while managing information and resources to achieve cost - effective, optimal patient outcomes as part of the Transition in Care for Justice-Involved Patients team.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES
- Identify and address patient needs / risk factors and / or obstacles to care, and available services and resources
- Develop a patient-centered case management plan including patient self-management goals; assess and address barriers to goals and connect the patient to services and resources to meet established goals
- Provide frequent reassessment, evaluation and direct patient care for our patient population into Optimus and document case management plans and on-going activities
- Communicate the care preferences of patients, serving as their advocate, and verifying that interventions meet the patient’s needs and goals of treatment; works with the entire health care team, including outside agencies to achieve the goals and expected outcomes for patients
- Collaborate and facilitate needed changes to the plan of care; ensure the ethical and legal issues related to patient care delivery are addressed and that care is provided appropriately
- Provide care team management, leadership and coordination
- Collaborate with LCSW and case managers to define patient’s needs and assist in plans for social service care; coordinate the patients' ongoing care in conjunction with outside agencies as needed
- Assist and advise the organization in the development of policies and procedures that will guide safe and effective quality of care outcomes; organize care team meetings and is an active participant in the development of quality care initiatives
- Participate in studies and research related to patient outcomes as well as studies that support evidence-based practices in health care
- Perform other duties as assigned
ADDITIONAL GENERAL REQUIREMENTS
Professional positive attitude, vision, understanding of customer service principals, trustworthiness, and excellent interpersonal skills to successfully accomplish tasks necessary to meet high standards of ethical and social responsibility required by this position. Must possess a valid current State of Connecticut Registered Nurse license.
JOB QUALIFICATIONS / REQUIREMENTS
EDUCATION : Graduated from an accredited registered nursing program approved by the State of Connecticut Department of Public Health with a Bachelor’s Degree.
EXPERIENCE : 3-5 years of relevant clinical / health care industry experience : Ability to work well with diverse and special populations; motivational interviewing techniques. Familiarity with tele-medicine & prior experience a plus
LANGUAGE SKILLS : Bi-lingual English / Spanish is helpful.
MATHEMATICAL SKILLS : Basic mathematical skills as it relates to medication calculations
REASONING ABILITY : Critical thinking skills, ability to create positive motivation. Strong verbal and written communication skills. Ability to work independently. Must have ability to organize and manage multiple priorities and work under time pressure deadline. Ability to demonstrate consistently sound judgment, accessibility and responsiveness to all team members. Self-starter who proactively and continually identifies more efficient processes and procedures.
