The Options Residential Community Supports Team provides services to adults living independently, helping them maintain and improve their behavioral health. Our primary goal is to support individuals in achieving their personal goals and fostering greater independence.
We have an exciting opportunity to join our team as a Housing Case Manager! This position is crucial in providing property and case management services to up to 20 individuals with disabilities in an independent living setting. Their primary goal is to help clients achieve housing stability and personal independence. This position advocates for clients' needs, ensuring they meet key expectations such as timely rent payments and positive relationships within the community.
What does a Housing Case Manager do?
The Housing Case Manager assists clients in developing skills across multiple life domains by employing teaching, coaching, and resource linkage strategies. The Case Manager addresses conflicts and emerging crises while guiding clients through problem-solving processes to prevent destabilization and maintain progress. In addition to individual interactions, the Case Manager is committed to fostering a safe and harmonious community environment. This position necessitates creativity, flexibility, and the practical application of best practices in the field. The Housing Case Manager must also manage a flexible schedule accommodating planned and drop-in sessions, requiring time management and organizational skills.
Main duties include:
Client Support & Case Management:
- Provide ongoing support to individuals living in permanent supportive housing and ensuring clients meet housing and personal goals.
- Link clients to essential community services, addressing challenges related to mental health, substance use, employment, healthcare, and more.
- Conduct regular client check-ins through scheduled and drop-in sessions, ensuring progress toward independence.
- Implement safety and emergency procedures to protect both clients and staff.
- Maintain a thorough understanding of available community resources to support client needs.
- Provide Person-Centered Case Management, lead Client Skills groups, and facilitate Community Support.
Property Management:
- Ensure the property's upkeep, including requesting maintenance services and maintaining common areas (e.g., vacuuming hallways).
- Educate clients on lease agreements, tenant handbooks, and other relevant housing policies.
- Perform monthly apartment inspections to maintain quality standards.
Community Engagement & Safety:
- Cultivate a safe and supportive housing environment by proactively managing client interactions and addressing emerging issues that may jeopardize housing stability.
- Facilitate conflict resolution and problem-solving to promote community harmony and individual well-being.
- Encourage responsible behaviors, such as timely rent payment, apartment maintenance, and fostering positive neighborhood relationships.
- Serve as a role model for clients, exhibiting professionalism, firm boundaries, and productive behavior in all service interactions.
Team Collaboration & Innovation:
- Collaborate with a multidisciplinary team to ensure holistic client care, sharing information and best practices to improve service delivery.
- Contribute creative solutions to enhance program effectiveness and client outcomes.
- Demonstrate flexibility and adaptability in responding to client's evolving needs and the dynamic program environment.
Documentation & Compliance:
- Maintain accurate, timely, and comprehensive documentation of client interactions and progress.
- Ensure compliance with federal, state, and organizational policies and regulations, including safeguarding personally identifiable information (PHI) by HIPAA standards.
- Maintain electronic health records and ensure timely billing for services rendered.
Starting Salary Range: Hourly position, $20-21 hourly DOQ.
Schedule: Mon-Friday, 40 hours weekly. Schedule is flexible to client needs, hours will typically be between 9am-5pm or 10am-6pm.
Benefits: Eligible 1st of the month after 60 days employment. Health insurance (ICHRA), Dental, Vision, Company Paid Life/LTD. 401k-eligible after 90 days employment. Must be scheduled 30 hours per week to be benefits-eligible.
Qualifications:
- High School Diploma or equivalent
- One year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older
- Strong communication and organizational skills, with the ability to work independently and take initiative.
Options Residential, Inc will conduct a Minnesota Department of Human Services Background Study on all new employees. Employees in driving positions will also be subject to a Motor Vehicle Background check. Continued employment will be contingent on a cleared background study and a satisfactory MVR.
