                                                                                  Summary 


This position is part of a Supportive Housing partnership between Simpson Housing Services, Aeon, and Avivo. The Case Manager will provide case management to residents at Aeon’s Opportunity Housing Program (OHP), which offers 143 formerly homeless adults a permanent home and support services to help them regain stability. Working within a team of three agency partners, the Case Manager supports participants in maintaining housing and accessing community resources. They will serve a caseload of 18 participants, addressing complex challenges such as mental health, chemical abuse, trauma, and criminal histories. 


The role requires close collaboration with the Simpson, Aeon, and Avivo teams, as well as maintaining strong relationships with local service providers. The Case Manager works primarily with residents at the Lamoreaux and Continental properties in downtown Minneapolis. Hours are typically Monday to Friday, 9:00 AM to 5:00 PM, with flexibility based on participant needs. 


                                                                         Program Goals 


  • Move clients into housing quickly to minimize time spent experiencing homelessness. 
  • Assist in removing housing barriers, connecting clients with resources like expungement services. 
  • Support clients in gaining independence, self-advocacy and stability. 
  • Work with individuals to maintain long-term housing. 


                                                                       Responsibilities 


Provide Quality Service to Participants: 


  • Welcome new residents, explain services, and offer support for accessing resources. 
  • Build supportive relationships to help residents achieve self-sufficiency. 
  • Conduct needs assessments and develop individual service plans with participants. 
  • Connect residents to local social services to meet basic needs and goals, including mental health crisis services and Adult Protective Services. 
  • Offer flexible services based on participants’ needs, including group shopping. 


Support Participants in Maintaining Housing: 


  • Screen and assess eligibility for Housing Support. 
  • Assist participants with paperwork to maintain housing. 
  • Mediate landlord-tenant conflicts and assist in crisis intervention to prevent eviction. 


Case Management and Crisis Intervention: 


  • Manage a caseload of 18-20 participants, meeting for at least an hour each week to ensure Medicaid billing and Housing Stabilization Services compliance. 
  • Maintain regular contact with participants to prevent crises and support their needs. 
  • Collaborate with team members and attend team meetings. 


Documentation and Recordkeeping: 


  • Maintain thorough, accurate and timely case notes and documentation in compliance with Hennepin County, Housing Support, and Housing Stabilization Services requirements. 
  • Keep records of time sheets, expenses, and required reports. 


Representation and Networking: 


  • Build positive relationships with staff, volunteers, participants, and community partners. 
  • Attend staff meetings, seminars and workshops related to professional development.
  •

  • Collaborate across departments to promote equity and antiracism. 
  • Participate in agency trainings and workshops on diversity, equity, inclusion, and antiracism. 


Required Skills and Knowledge: 


  • Ability to work independently and as part of a team. 
  • Strong time management and organizational skills. 
  • Familiarity and alignment with the Harm Reduction/Housing First model, trauma-informed services and motivational interviewing techniques. 
  • Ability to work with individuals from very diverse backgrounds. 
  • Ability to perform non-traditional tasks while meeting participant needs. 
  • Excellent communication skills with diverse populations, including participants, Property owners, and service providers. 
  • Strong computer skills, with proficiency in Microsoft Word, Excel, and Outlook. 
  • Ability to maintain confidentiality. 
  • Adhere to a standard framework of boundaries in all interactions with guests, volunteers, staff and public. 


Job Requirements 


  • Bachelor’s degree in human services or equivalent experience. 
  • Knowledge of housing resources and services. 
  • Valid driver’s license and access to a car to use for work. 


Desired Qualifications 


  • At least 1 year of experience in social services, particularly with low-income individuals, people experiencing homelessness, and those with mental health or substance abuse issues. 


Physical and Mental Requirements 


  • Ability to empathize and problem solve, maintaining openness to diverse perspectives. 
  • Must be comfortable with home visits and personal safety awareness. 
  • Ability to lift up to 25 pounds and move around community spaces including stairs. 


Work Environment 


  • 80% time spent indoors, 20% outdoors, with 5% travel for community meetings and participant appointments. 
  • 80% of the role involves direct participant care.
