Position Overview
Support Consultants provide education and guidance to individuals participating in the Community First Services and Supports program. They assist in writing service delivery and quality review plans, evaluate service utilization, and offer recommendations to improve the quality of services. Meetings with participants may take place over the phone or in their homes.
Duties & Responsibilities :
- Conduct home visits to educate program participants on program models, rules, processes, and policies.
- Assist with writing service delivery plans and quality review plans.
- Evaluate service utilization according to approved plans and provide recommendations for improvement.
- Organize and manage a caseload effectively, ensuring compliance with contractual timelines.
- Maintain timely documentation, ensuring visit notes are completed and uploaded on the same day.
- Represent the organization positively to participants and stakeholders.
