What are the responsibilities and job description for the Community Health Aide position at Orange County Government?
Orange County Health Department is seeking a Community Health Aide . This position is benefited, time-limited position. The work involves the responsibility for providing outreach, counseling, and navigational case management services to formerly incarcerated individuals with chronic medical, psychiatric and substance use issues. An employee in this class is responsible for identifying eligible participants at the local detention and/or prison transition site prior to their release, establishing rapport, encouraging participation, and creating comprehensive reentry plans with participants.
Work is performed under the general direction of a Public Health Services Manager with leeway to exercise independent judgment on a limited basis with regular guidance provided by the Public Health Services Manager, providers and the Community Health Services Director within the guidelines of established protocol and North Carolina Board of Health regulations governing the responsibilities of County Health Departments within the framework of applicable rules, regulations, policies, and procedures in the performance of duties, both in terms of planning and accomplishing work, and in making decisions regarding customer needs and requests and conducting project assignments. Supervision over others is not required of this position. Position does related work as required.

MINIMUM QUALIFICATIONS
Graduation from High School or possession of Graduate Equivalency Diploma (GED) and one (1) year of work experience providing outreach, counseling, and case management services and personal history of incarceration.
An equivalent combination of training and experience as defined above. A higher degree does not substitute for work experience.
SPECIAL REQUIREMENTS
Possession of or ability to readily obtain a driver’s license valid in the State of North Carolina for the type of vehicle or equipment operated.
All offers of employment are conditional upon successful clearance of a background check. Employment cannot commence until the employee has been cleared.
Salary : $37,931 - $43,000
