Job Title: Care Coordinator - Bilingual
Location: Des Moines, IA (Downtown)
Pay for this position is posted at: $20.25
Schedule for this position: 8:00am to 5:00pm, occasional evenings. Opportunity to work from home 2-3 days per week.

Position Overview: The Care Coordinator provides care coordination in accordance with the pediatric integrated health home model. The Care Coordinator plans and coordinates care based on child/youth member needs; works as an integral member of the care team to ensure family and program outcomes are met; and coordinates client care across providers. This position requires the ability to manage time effectively due to high caseload demands and to work collaboratively on a team that includes a nurse care manager and family peer support.
Key Responsibilities:
- Connect families to community resources
- Serve on multidisciplinary treatment teams on behalf of families and children
- Help families identify meaningful ways to deescalate and calm
- Share best practice strategies for family wellness
- Plan and coordinate care based on the needs of the child, youth and family
- Evaluate the effectiveness of care provided
Qualifications:
- BA degree in social work or human services related field
- Bilingual in Spanish and English
- Case management experience with children, youth and families preferred
- Strong organizational skills to maintain high caseloads with care team support
- Excellent communication and interpersonal skills across multiple providers and perspectives
- Ability to engage families from diverse backgrounds
- Desire to learn about and develop working knowledge of other service systems
- Excellent computer skills
