Job Description
This role involves providing high-quality nursing care to patients in their homes. You'll conduct thorough assessments, develop personalized care plans, and work collaboratively with healthcare professionals to ensure seamless care.
Responsibilities:
- Deliver direct nursing care, including wound care, medication administration, and chronic disease management
- Teach patients and their families about health management and self-care practices
- Maintain accurate and timely clinical records in compliance with regulations
Requirements:
- Licensed RN: Current and unrestricted RN license in MA
- Preferred experience: At least 1-2 years of nursing experience, preferably in home health. Willing to train the right person
- Flexibility: Ability to work independently and adapt to varying patient needs and schedules
- Strong communication skills: Excellent interpersonal skills to effectively interact with patients and their families
