Job Summary:
The Juvenile Diversion Case Manager will undertake a variety of case management tasks related to diversion programming in the Juvenile Division, including, but not limited to, organizing, preparing, planning and implementing the service plans for children referred to diversion and monitoring the participants' progress thorough the completion of the program. The Juvenile Diversion Case Manager is responsible for scheduling intake meetings with all juvenile diversion referrals and their families, completing all diversion documentation with participants and families, and creating a coordinated plan of services for the participant.

up to $55,000 (grant funded)
Duties and Responsibilities:

Manage and respond to incoming diversion referrals.

Complete intake meetings with all diversion referrals, complete all diversion documentation with participants and families, and complete a plan of coordinated services for each participant.

Complete risk, trauma, and mental health assessments with referred children.

Create case plan for each referred child including deadlines, milestones, and processes.

Effectively manage a caseload of 30 - 40 participants.

Implement evaluation strategies to monitor participant performance and to determine the need for adjustments/improvements.

Ensure case plan always supports and advances the juvenile diversion program's mission.

Maintain records and tracking system for juvenile diversion participants.

Build positive relationships between juvenile diversion and community partners.

Assist with identifying community resources that could potentially partner with OPDA.

Participate in discussions related to possible grants related to juvenile diversion.
Required Qualifications and Experience:
- Master's Degree in Social Work, Counseling, or a related field (licensure preferred, but not required)
- 3 years minimum working with children and families
- Excellent organizational and leadership skills
- Must have the ability to engage parents and children
- Must have great problem-solving abilities
- Must be organized and detail-oriented
- Must have exceptional communication skills, both oral and written
SALARY AND BENEFITS:
Salary commensurate with experience up to $55,000 (grant funded).
New hires may be eligible for:
