Expected pay for this position is $33.85 - $39.83/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
Overview
POSITION SUMMARY:
The Case Manager (CM) I works in collaboration with the patient/family, physicians, and multidisciplinary team members to ensure patient progression through the continuum of care and to independently develop a plan of care for each assigned patient from admission through discharge with minimal direction. Responsible for identifying, initiating and managing optimal patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety, and length of stay management. Utilizes their knowledge and expertise to evaluate the individual patient's needs via comprehensive assessment and managing care coordination and discharge planning. The CM assesses and responds to patient/family needs by coordinating efforts of the interdisciplinary team members and identifies and resolves barriers that hinder effective patient progression of care. Ensures all communication is appropriate, accurate, timely, and consistent with the patient’s plan of care.
Qualifications
REQUIRED QUALIFICATIONS:
Education:
Registered Nurse with 1 year healthcare setting experience (Bachelor's degree of Science in nursing within three years of hire)
OR
Bachelor's degree in social work or social services with licensure and 1 year of healthcare setting, discharge planning, care coordination, or OSF Case Management Internship
Licensure/ Certification:
Registered Nurse License (RN),
OR
Social Worker License (LSW), Licensed Bachelor Social Worker (LBSW) or Licensed Professional Counselor (LPC) in IL or MI.
Other Skills/ Knowledge:
Solid computer skills, including proficiency with Microsoft software.
Strong analytical and problem-solving skills, with the ability to be detail oriented.
Excellent interpersonal and verbal and written communication and ability to interact with diverse populations.
Strong critical and analytical thinking skills.
Ability to multitask and prioritize in a fast-paced environment.
Ability to demonstrate a high level of emotional intelligence.
Working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement.
Basic understanding of pre-acute and post-acute levels of care and community resources.
Ability to work independently and exercise sound judgment in interactions with physicians, payors, patients and their families.

PREFERRED QUALIFICATIONS:
BSN or MSW with licensure and 1 year of healthcare setting discharge planning, care coordination, or OSF Case Management Internship
Licensure/ Certification:
Clinical Social Worker License (LCSW) or Licensed Master Social Worker (LMSW) or Clinical Professional Counselor License (LCPC) in IL or MI
