Otterbein Hospice compliments already strong independent senior services, outpatient capabilities and clinical facilities at Otterbein.

Responsibilities
Otterbein is seeking an enthusiastic and professional Nurse Practitioner.
- The role of the Nurse Practitioner is to provide care to the terminally ill Hospice patient as needed. To provide assistance and understanding to the family in the home care situation and in time of bereavement. To work as a member of the Hospice team in providing Hospice care.
- Participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties
- Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR
- Organizes work schedule and utilizes time management to be able to attend all required
- Complies with Hospice infection control policies and protocols
- Assist with orientation, teaching and training as requested
- This role is responsible for primary caseload.
- This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws and agency policies and procedures.
- Education: Graduate from an accredited school of nursing. Nurse Practitioner is an advanced practice RN and a type of mid-level practitioner who is educated at the graduate level to provide primary, acute chronic and specialty care. Obtains graduate education at the master's, post-master's or doctoral level and obtains national board certification.
- Licensure/Certification: Valid Ohio Certified Nurse Practitioner (CNP) license
- Experience: Minimum 2 years of experience as a registered nurse practitioner, preferred. Active patient contact within past three years, preferred.
