Job Description
As the Arizona Intake Specialist for Outreach Health, you are responsible for performing initial client intakes, which includes interviewing clients, assessing the client’s needs, immediate staffing, and providing client care. Because the Intake Specialist is the first voice a new client will hear, you become the director of first impressions for Outreach Assured Care with the responsibility of providing exceptional customer service and support to clients seeking services. The Intake Specialist must have strong organizational skills, excellent communication and interpersonal skills, and the ability to think critically and provide appropriate solutions. This position requires the ability to work independently, within a team and always maintain a professional demeanor, even in difficult times. This position requires daily driving and will be reimbursed for mileage.

Essential Job Functions
- Must have reliable transportation as traveling valley wide is required
- Initial Client Intake visits (in client's home)
- Notify the MCO CM once intake is completed to receive authorization of services
- Once client is on boarded into the system: Create Care Plans for all clients
- Provide immediate care to get the cases started
- Assist with any immediate schedule changes for all clients
- Document any issues and/or positive information in the Client Logging and/or Caregiver Logging system in EMR as appropriate
- Updates information as needed in a timely manner within our EMR system
- Update Excel Workbook trackers - Client related
- Report any concerns to the Office immediately via email
- May have to provide care for clients at any given time due to call off's if not able to restaff with a caregiver
- Any other duties as deemed necessary by the Director
- After 90 days: qualifies for the On Call Rotation - additional stipend while on call

Required Skills, Education, Certification, License, and/or Experience
- High School graduate or GED
- Excellent computer skills
- Must be detail oriented and show attentiveness to accuracy
- Have good time management and problem-solving skills
- Strong customer orientation with excellent interpersonal and communication skills
- At least one (1) year of caregiving experience is required
- Must pass a background check, as well as the ability to obtain Fingerprint Clearance Card
- High level of integrity and dependability with a strong sense of urgency and results orientation
- Must be humble, teachable, and open minded
Preferred Skills, Education, Certification, License, and/or Experience
- Experience in Home Health, Hospice, and/or Home Care
- Bilingual is preferred (English and Spanish)
Working Conditions and Physical Requirements
While performing the duties of this job, the employee may encounter light exertion in lifting, moving, and carrying lightweight supplies and equipment. Must be able to work in a work environment that involves intermittent body movements such as walking, standing, bending, stooping, kneeling, reaching, sitting, and viewing monitor(s) for extended periods. Duties involve application of skill requiring motor coordination, finger dexterity and strength in hands/limbs for typing, filing, machine operation, and or a reasonable accommodation to perform required tasks. The Nature of duties presents moderate exposure to infectious and chemical hazards or job-related accidental injury.
