What are the responsibilities and job description for the Intake Coordinator position at Overlook Visiting Nurse Association Inc.?
POSITION SUMMARY
The Intake Coordinator is trained and experienced in referral management with relevant experience and knowledge in the home health field.
This position is full-time, and the schedule is Monday through Friday with occasional weekend coverage as needed, as well as 2-3 major holidays per year.
PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:
Responsible for collecting home care referral information and entering data into the EMR.Assists with phone management and customer service.
Assists with scheduling on the weekend.
Assists with following/tracking active clients that have been transferred inpatient.
Works closely with field community liaisons to track/monitor referral activity.
Communicates daily with internal staff to obtain necessary data for dissemination to referral sources (branch availability by discipline, territory).
Maintains confidentiality of patient health information.
ü Adheres to OVNA and MHS policies and procedures.
ü Additional responsibilities as assigned by the Director of Patient Access.
POSITION REQUIREMENTS:
Be an LPN/LVN licensed in the state in which he/she practices, with at least one year of clinical experience in a healthcare setting.
Must have excellent organizational, communication, and interpersonal skills.
Computer proficiency.
Experience in Customer Service or Hospitality - preferred
