What are the responsibilities and job description for the RN Team Manager PCM/CCM Program position at Owensboro Medical Practice?
GENERAL SUMMARY OF DUTIES: Medical Practice seeking self-motivated, mature individual to plan, direct, or coordinate medical services between patient and office. Responsible for direct and indirect patient care under the direction of the provider(s). Responsible for ensuring efficient patient services complying with established policies and procedures of CCM/PCM program.
ESSENTIAL FUNCTIONS:
1. Directs and supervises nursing services and diagnostic services in accordance with governmental and other regulatory standards.
2. Facilitates development of department mission, goals, policies, procedures, budget, work standards.
3. Compiles/analyzes data, prepares and presents statistical information on department performance, productivity, activity, and budget to administration.
4. Develops administrative processes/methods to improve performance.
5. Oversees nursing care essential to the assessment, promotion, maintenance and restoration of patients' health and wellness.
6. Oversees patient needs and suggestions solutions to patient care problems.
7. Selects, orients/trains, develops, and evaluates staff.
8. Ensures the development and delivery of nursing education to staff.
9. Maintains patient confidentiality.
10. Working knowledge and application of Electronic Health Records (EHR).
EDUCATION: Associates of Science in Nursing from accredited school of nursing; BSN preferred.
EXPERIENCE: Case management experience preferred. Minimum of two years professional nursing experience. Minimum of two years of experience with use of an EHR system. Experience/knowledge of process improvement methods a plus.
LICENSURE REQUIREMENTS: State RN license.
KNOWLEDGE/SKILLS/ABILITIES:
1. Knowledge of management principles involved in strategic planning, resource allocation, leadership, and coordination of people and resources.
2. Knowledge of medical safety practices and requirement to evaluate existing standards and implement new procedures.
3. Knowledge of current nursing practices and new nursing practices and medical advances.
4. Application of the principles, methods, and techniques of professional nursing.
5. Identification of problems, researching, and recommending resolutions.
6. Developing and maintaining department quality assurance.
7. Exercising a high degree of initiative, judgment, discretion and decision-making with a bias toward action.
8. Ability to communicate clearly both orally and in writing.
ENVIRONMENTAL/WORKING CONDITIONS: Frequent exposure to communicable diseases, medicinal preparations, ionizing radiation, and other conditions common to a clinic environment.
