We are seeking a Social Worker - Discharge Planner.

• $70K to $72K annually based on skills and experience.

Social Worker - Discharge Planner Qualifications:  

• Minimum of 2 years’ experience in a Skilled Nursing Facility (SNF).
• Master’s in Social Work or related healthcare field.
• MDS and Short-Term Discharge planning experience required.

Social Worker - Discharge Planner Introduction: 

The Social Worker - Discharge Planner will collaborate with the Director to plan, organize, and oversee the Social Services Department's day-to-day operations, ensuring the highest level of care and support for our residents.

Social Worker - Discharge Planner Job Summary: 

• Plan discharge and referring residents to appropriate services.
• Conduct interviews with residents and families to gather social histories.
• Provide solutions for social and environmental challenges, including financial assistance, discharge planning, and referring to community agencies. 
• Complete all documentation in the electronic medical record - MDS. 
• Schedule care plan meetings.
• Education of the care team and completion of advance directives.
• Coordinating social service programs with administration, medical and nursing staff, and other departments.
• Participate in state and federal inspections and surveys as needed.
