What You’ll Do
- Personalized Resident Care: Manage a caseload of 80 residents, ensuring personalized social and emotional support.
- Care Planning: Develop and implement comprehensive care plans through your knowledge of the Minimum Data Set (MDS) and care planning process.
- Facilitate Smooth Transitions: Guide residents and their families through discharge planning, ensuring safe and smooth transitions back to the community or other care settings.
- Family & Team Collaboration: Lead weekly family and interdisciplinary team meetings to discuss progress, address concerns, and make care recommendations.
- Charting & Documentation: Maintain accurate and timely records of resident interactions, care plans, and progress notes.
- Resource Advocacy: Connect residents with community resources and services that enhance their well-being.

Job Details
- Job Type: Full-time (Monday - Friday, 9 AM - 5 PM)
- Special Focus: Traumatic brain injury and geriatric care
