Job Summary:
Seeking a compassionate and skilled Mind at Home/Service Coordinator to assist individuals with dementia or Alzheimer's in accessing waiver services, MA State Plan services, and other medical, social, and educational services. This role requires exceptional customer service skills and a strong desire to help others. Monday-Friday 8:30 A.M.-5:00 P.M. 60% On the road conducting quarterly face-to-face visits (case load up to 50 participants) 40% Remote work from home.
Responsibilities:
- Conduct person-centered assessments and memory care evaluations quarterly.
- Collaborate with hospitals, social workers, nursing homes, and adult protective services for care plans and discharges.
- Develop and modify service plans for participants.
- Assist participants in finding approved service providers.
- Educate participants and family members on brain health and caregiver burnout prevention.
- Attend Mind at Home care plan meetings weekly and Telecollaborative meetings biweekly.
- Coordinate with agencies to obtain necessary services.
- Evaluate participant needs and document all work completed.
- Assess funding sources before submitting requests.
Requirements:
- Bachelor's degree, preferably in social work or psychology.
- Prior service coordination experience is a plus.
- Knowledge of cognitive impairments is a plus.
- Proficiency in Microsoft Office and internet navigation.
- Strong problem-solving and prioritization skills.
- Availability for emergency situations and field visits.
