Responsibilities
- Create and manage individualized care plans for clients based on their specific needs and preferences.
- Coordinate daily schedules for caregivers and ensure adequate staffing levels to meet client requirements.
- Act as the primary point of contact for clients and their families, addressing any questions or concerns related to care services.
- Conduct regular assessments and evaluations of clients' conditions and update care plans accordingly.
- Ensure compliance with all relevant regulations and standards in home care services.
- Facilitate training and ongoing support for caregivers to promote best practices in client care.
- Maintain accurate and up-to-date client records, including care plans, service notes, and billing information.
Requirements
- Prior experience in home care coordination or healthcare administration is preferred.
- Strong communication and interpersonal skills to effectively interact with clients, families, and staff.
- Excellent organizational skills with the ability to multi-task and prioritize effectively.
- Proficiency in using HHAexchange, or at least being computer savvy to quickly learn it.
- Knowledge of local and federal regulations governing home care services is a plus.
- Valid driver's license and reliable transportation are required for client visits.
Job Types: Full-time, Part-time
Pay: $19.00 - $22.00 per hour
Expected hours: 20 – 40 per week
