What is a PACE program?

The Program of All-inclusive Care for the Elderly (PACE), is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.

Responsibilities will include but not limited to:

The Director of Clinical Services provides supervision and leadership to all Clinic and Home Care staff, coordinates all Clinical and Home Care services, has Clinical practice oversight, coordinates medical operations, and integrates Clinical and Home Care practices with the IDT. Responsible for ensuring compliance with all organizational policies and regulatory rules, as it relates to clinic operations. Provides leadership to the clinic team in regard to the PACE Model of Care and is directly involved in the QAPI process.

· Helps establish/implement goals, objectives, policies, procedures, and systems for the Clinic and Home Care.

  • Determines system for intradepartmental functions in keeping with Clinic and Home Care systems and communicates it to staff. Monitors effectiveness
  • Oversees and provide backup coverage for Clinical and Home Care staff.
  • Assist developing and implementing written policies/procedures/protocols governing the operation of the department.
  • Assist in developing quality improvement initiatives and work closely with Quality Director.
  • Knowledge of legal issues in all aspects of operations to ensure compliance with regulatory agencies and rights of all customers
  • Work closely with Center Manager and Quality Manager in developing and implementing policies and procedures to meet state and federal regulations.
  • Assists physicians with the development activities
  • Accesses data to effectively manage clinic operations by utilizing quality improvement tools.
  • Sets and communicates clinic standards in keeping with regulatory agencies and Health System policies.
  • Develops and monitors standards to comply with safety, infection control and customer relations standards.
  • To help communicate, and enforce the Health System's safety process, which includes department specific plan, emergency preparedness, hazardous materials/waste programs, security management plan, life safety/management (fire plan), equipment management, and utilities management.

Education and Experience:

· Bachelor of Science in nursing required, Master’s degree preferred.

· Minimum of one year of experience working with the frail, elderly, or long-term care population.

· Must have an active RN licensure in the State of Michigan.

· Must have reliable transportation; current valid Michigan driver’s license preferred.

· Possess current CPR certification by AHA or willing and able to obtain as offered by PACE Central Michigan.
