POSITION: Pre-Release Coordinator

HOURS: Part-Time and Full-Time positions available

ORGANIZATIONAL OVERVIEW

For over 60 years, Public Advocates in Community re-Entry (PACE) has provided resources and services to assist justice-involved individuals and their families to lead productive and responsible lives in our community. Our team believes that felonies don’t define futures, and that no one’s life should be defined by their worst day.

POSITION DESCRIPTION:

This position focuses on preparing individuals for release from incarceration by facilitating workshops, providing pre-release services, linking individuals to a range of community-based services, and utilizing PACE’s care-coordinated model for case management services to ensure all clients receive the services necessary to ensure a successful re-entry upon release from incarceration.

POSITION RESPONSIBILITIES:

· Facilitate an orientation and trauma-based Moral Reconation Therapy (MRT) curriculum within correctional facilities.

· Provide care coordination (work readiness, occupational training, individualized service plan development, occupational training, wrap around case management, etc.) to assist clients address potential barriers to success before their projected release.

· Ensure services continue upon release from the correctional facility and provide follow-up with clients a minimum of every 30 days to ensure long-term client engagement.

· Document all contact and progress in case management system and in client’s file.

· Provide support in a positive manner through coaching, role modeling and mentoring to assist members in developing skills to achieve their goals.

· Encourage clients to be actively involved in their own plan, exhibiting client-centered principles.

· Able to share personal or professional re-entry experiences and can model effective coping techniques.

· Find creative and effective solutions to complex problems and the ability to prioritize as appropriate.

· Maintain confidentiality and standards of ethical practice including HIPAA requirements.

· Must understand that it is the responsibility of every PACE staff member to provide client safety as a priority response according to evidence-based practices.

· Share success stories monthly regarding clients enrolled in the program.

· Provide written narrative monthly on the barriers faced by both the project and the clients.

Qualifications and Skills:

· Must have personal and/or professional experience with the criminal justice system and understand the unique needs of the justice involved population.

· A degree in Criminal Justice or related field is preferred but not required.

· Knowledge of community resources and recovery support systems.

· Clear understanding of how to address barriers to successful re-entry and services as a coach and an advocate.

· Strong interpersonal skills with the ability to work within a team environment as well as independently.

· Must have strong verbal and written communication skills with diverse audiences and purposes.

· Experience with data collection and reporting including the utilization of Microsoft Excel.

· Must be willing and able to travel to IDOC facilities around the State.

· Must have:

o A Valid Driver’s License;

o Valid Car Insurance; and

o Reliable transportation.
