Contract Length: 13 week contract
Start Date: ASAP
Guaranteed Hours Per Week: 36 hours
Primary Location: ThedaCare Juliette Manor
Shift Requirements: 12hr NOCs
**any missed shift must be made up within 2 weeks of the absence
Weekends: Every other
Holidays: As needed
Float Requirements: Throughout facility within skillset, To all ThedaCare Facilities (within a 60 mile radius)
On Call/Call Back Requirements: None
Certifications: WI CNA, AHA BLS, Medication Administration Cert (strongly preferred)
Minimum Experience: 2 years LTC experience REQUIRED
Recent LTC facility experience REQUIRED
Charting Experience Required: EPIC preferred & Pyxis preferred
Travel experience required.
Education Requirements: High School Diploma
Required Skillset: Worked in LTC, Post Acute, Dementia Care. Proficiency in skillsets including Peri Care & Cath Care.
Unit Information: 37 beds (20 beds LTC, 17 beds Rehab))
Patient Ratio: 1:10-1:15 (RN patient ratio 1:17-20, however maybe 1 RN from midnight - 530 at times)
Scrub Colors: Any
Orientation: 4 days total (1 day for onboarding & 3 shifts with preceptor)

GSA hours are not guaranteed the week of Orientation. Traveler is guaranteed set hours per week, except during holidays where unit is closed.
