What are the responsibilities and job description for the Intake Coordinator position at Pacific Homecare Services?

General

· Enhance and maintain the company culture by working and demonstrating the company’s core values

· Manage incoming calls and other communications

· Maintain confidentiality of our clients and care providers in compliance with HIPAA

· Attend quarterly staff meetings

· Perform other job-related duties as assigned

Service

· Conduct welcome calls to all newly referred families

· Send the welcome email to confirmed Agency Respite families

· Conduct intakes for Agency Respite families for all areas served in a timely manner

· Update the record for the individual served in the database, ensuring accuracy and validity of information

· Send and collect the enrollment documents for all Agency Respite families: Respite Service Agreement, Home Safety Checklist, and Emergency Plan

· Generate private agreements for sibling care and/or additional private pay hours for the individual served

·Update the Placement Manager on any issues/concerns regarding intake information.

·Coordinate and schedule Parent Orientations between parents and the Team Lead.

· Assist and support with Agency Care Provider appreciation events.

Qualifications

· High School Diploma with 1 year of experience.

· Strong computer skills, i.e., navigating between different programs and network.

· Excellent written and verbal communication skills.

· Data entry experience.

· Must be computer savvy and proficient in Microsoft Word, Excel, and Outlook.

· Bilingual in Spanish/English is preferred but not mandatory.

Job Type: Full-time

Pay: $25.75 - $27.50 per hour

Expected hours: 40 per week
