JOB DESCRIPTION SUMMARY:

Job Title/Position: Home Health Clinical Assistant Manager

The LVN Clinical Assistant Manager is a professional, licensed nurse responsible for assisting with coordinating and managing patient care appropriately. The Clinical Assistant Manager is responsible for supporting and aiding the Director of Patient Care Services (DPCS), Clinical Manager (CM) as well as the clinical field personnel to ensure that care and services are delivered appropriately.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

  • Assist the home health department with various administrative clinical tasks, including but not limited to placing and answering phone calls, receiving and generating reports, writing correspondence, contacting MD to obtain orders or clarify orders.
  • Processing orders, lab results and or requests received via telephone, fax, or email.
  • Receives and processes incoming clinical fax documentation.
  • Receives case referrals. Reviews available patient information related to case, including disciplines required, to determine home health needs.
  • Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs, and guides clinicians to promote more effective performance and delivery of quality home care services, and is always available during operating hours to assist clinicians as appropriate.
  • Reviews patient’s clinical diagnosis, medications, procedures, and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plans of care.
  • Attends case conference meetings with organization personnel to facilitate coordination of care.
  • Assists with conducting quarterly record reviews and communicates findings and recommendations to DPCS and appropriate organization personnel.
  • Assists in the orientation of new organization personnel.
  • Promotes customer service orientation to all organization personnel.
  • Assists DPCS in the planning, implementation, and evaluation of in-service and continuing education programs.
  • Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
  • Participates in Quality Improvement and Corporate Compliance activities as assigned. Assists with other chart audit activities as assigned.
  • Performs other duties and activities as delegated by the Clinical Director

POSITION QUALIFICATIONS:

  • Licensed Vocational Nurse with current license to practice professional nursing in the state.
  • One (1) to two (2) years of clinical home health experience required.
  • Knowledge of federal regulations and state licensure requirements.
  • Working knowledge of computer systems.
  • Possess and maintains current CPR certification.
  • Maintains routine medical screening requirements in accordance with State Department of Health for nursing/healthcare workers.
  • Licensed driver with automobile that is in good working order and insured in accordance with organization requirements.
  • Excellent coordination and communication skills.
  • Detail oriented and able to work with minimal supervision

  • Sitting at a desk for 6-8 hours per day.
  • Keyboarding and mousing for 6-8 hours per day.
  • Standing or Walking about the office for up to 1 hour per day.
  • Speaking on the phone for 1-3 hours per day.
  • Listening/Hearing in person speech, telephone, and other typical office noises 6-8 hours per day.
  • Will endure multiple interruptions throughout the day, must be successful at refocusing on the task at hand.
  • Near Visual Acuity using a computer 6-8 hours per day.
  • Lifting and carrying up to 15 lbs. on occasion.

Job Type: Full-time

Pay: $35.00 - $37.00 per hour
