What are the responsibilities and job description for the Patient Navigator position at Pacifica Hospital of the Valley?
The Patient Navigator assists patients with access to evidence-based treatment for substance use disorders (SUDs) with a focus on low-threshold access to medication for addiction treatment (MAT) and harm reduction. The Patient Navigator conducts outreach to patients identified by the Emergency Department, conducting initial brief patient interviews, introduces patients to MAT programs and services, facilitates appointments with outpatient MAT clinics, serves as a resource and coach for their clients, advocates for culture change in the hospital. The Patient Navigator may also assist with the access to other services, such as mental health services, primary care, social services, and residential treatment facilities.
