About the Position
Under the direction of the Chief of Operations, the Program Coordinator is responsible for supervising a small team of case managers, community health workers, or other qualified staff in providing comprehensive case management to identified populations of focus enrolled in Enhanced Care Management (ECM) or other case management programs at Pajaro Valley Prevention and Student Assistance (PVPSA). The Program Coordinator will work alongside Executive leadership to develop and maintain the organization’s care management operations to ensure that individual youth with complex care needs receive both the clinical and non-clinical supports available. The coordinator will also provide a minimum of 10 hours of direct ECM services to children and/or families weekly.

Duties & Responsibilities
Oversees care management programs that address the complex needs of clients, provide clinical and non-clinical care coordination, and meet PVPSA’s organizational goals
Works closely with other PVPSA departments in coordinating program referrals, implementation, ongoing monitoring, and reporting
Outreach and engage with community, school, county, and/or CCAH partners including Community Supports
Ensure standardization and optimization of workflows, staff training on documentation guidelines and Community Supports referrals, and timeliness of billing through a designated EHR
Review and assign new referrals by staying abreast of staff caseloads. May include coordinating referrals to other resources within or outside of PVPSA and ensuring services are rendered (closed loop referrals)
Support staff with routine caseload consultation and supervision, staff meetings and ensure attendance at required and recommended training
Performs miscellaneous job-related HR and supervisory duties as assigned
Direct care management service delivery to eligible children and their families. Duties include but are not limited to:
Conduct outreach, assessment, and care planning of referred children, adolescents and their families in a school, home or community-based clinic setting
Submit Youth ECM Provider and Community Support referrals on the CCAH website and bill services in a timely manner through designated EHR following documentation guidelines
Complete paperwork at required frequencies (consents, releases of information, progress notes, assessments, care plans, etc.)
Connect with client families, school personnel, physical and behavioral health providers, and supporting agencies to best coordinate client care. Provide wrap around support
Work with other members of the PVPSA treatment team (supervisors, MHRS, case managers, CHWs, crisis, therapist, SUD counselors, etc.) to provide individualized, effective support
Participate in staff meetings, in-service trainings, conferences and workshops
Adhere to all confidentiality requirements and mandated reporting laws

Training and Experience
Requires at least an AA degree or CHW certification with 1 years of supervisory/leadership experience
Experience working in a community-based setting for at least 1 to 2 years preferred
Bilingual skills highly desired (Spanish/English)
Annual Salary Range
$60,652.80 - $66,869.71 commensurate with experience. Eligible for medical, dental and other employee benefits.
