The PALM Clinic Nurse l Patient Liaison is a patient education and care coordination role responsible for facilitating patient care and coordinating medical, wellness, and complementary service needs for PALM members. The Clinic Nurse will be the point person for a designated physician (or physicians) and will work to coordinate care for that physicians patient panel by means of prepping the physician prior to the appointment, smoothly managing the physician's schedule during the day, communicating changes or needs to the physician and any other team members as necessary, and providing patient education. The Clinic Nurse is responsible for educating and coordinating care for patients following their appointment and will assist with any further needs within and outside of PALM. This could include, but is not limited to, scheduling future appointments, gathering needed supplements, describing lab collection instructions, relaying messages from the physician to the patient and vice versa, as well as providing education on lifestyle, diet, and medical recommendations. The Clinic Nurse serves as the back-up for other nurses, medical assistants, and administrative roles as needed. Excellent client service and hospitality are at the heart of this role.
Required Qualifications
- Bachelors degree or higher in nursing (required)
- Two years medical office experience (required); will consider other combinations of education and experience
- Licensed Registered Nurse in the State of practice (required)
- Completion of course work in medical terminology and medical records (preferred)
- Phlebotomy training (preferred)
