The position of Individual Support Coordinator plays a vital role in the relationship to the interdisciplinary team and the support services provided to each person. The ISC provides continuous and active involvement, which includes an assertive and consistent implementation of an Individuals Service Plan, based on each persons strengths and assessed needs. The Individual Support Coordinator (ISC) is responsible for oversight of supports and services to individuals in their case load. The ISC will monitor and document program execution at the site while also acting as a liaison for the Program Director. The ISC will be responsible for overseeing incident reporting procedures with individuals on their case load. He/she will oversee and monitor all program logs and documentation. The ISC will be expected to maintain and model, at all times, appropriate behavior and interactions for staff and clients. The ISC will participate at all mandatory staff meetings. The ISC will update the Program Director and staff of any relevant issues pertaining to individuals on their case load. The ISC is responsible for coordinating all supports, services and activities for clients when appropriate. The ISC will be responsible for ongoing process improvement recommendations. The principal functions of the position identified shall not be considered as a complete description of all the work requirements and expectations that may be inherent in the position.
POSITIONS ESSENTIAL FUNCTIONS
- Maintains communication with staff and observes staff to monitor program strengths and weaknesses
- Knowledge of Developmental Disabilities and reporting procedures in their local Counties.
- Sufficient knowledge and experience to administer effective and ongoing operations of the Developmental Disabilities services.
- Skill in providing supervision
- Must have a current and valid Drivers License
POSITIONS ESSENTIAL FUNCTIONS
- Oversee the coordination of transportation needs to & from the Day Program with the Transportation Coordinator and Program Director.
- Initiates the collection of daily attendance and reports changes in attendance as necessary to the Program Director.
- Ensure Individuals are receiving meaningful programing that is scheduled and facilitated and reporting to the Program Director.
- Responsible for reviewing and ensuring all documentation is completed timely and accurately reflects the days programing. This includes, but is not limited to, transportation, billing, and program documentation for individuals in their case load.
- Responsible for the collection, review and submission of unusual incident reports. This includes the assessment of risk to the individuals health & safety and ensures proper notification and plan of action is implemented and reported to the Program Director.
- Contributes to and presents at weekly staff meetings.
- Promotes a supportive environment by modeling the guidelines of company philosophy, confidentiality, people-first language, empowerment, and fair and reasonable treatment of others.
- Responsible for building and maintaining professional relationships with community to assist in program planning.
Teamwork, Professionalism, and Communication
- Functions as an active team Leader by, reporting and contributing to Individual Service Plan meetings and reporting to the Program Director.
- Implements and follows company policy and procedures and information according to code of conduct / employee handbooks and other related materials
- Assist with individuals served, and general in-service/training for new employees and other support staff as requested by the Program Director.
- Demonstrates professionalism within the program site and the community, in action and appearance.
- Communicate effectively and respectfully with team members in a clear, understandable voice, free from derogatory terminology or overtones.
- Promotes positive family, neighbor, and community employment relationships-and informs Program Director of ALL concerns or complaints.
- Completes task/responsibilities independently and without needed additional reminders.
Support Services, Skill Development, Assistance & Supervision, and Behavior Plans:
- Oversees the consistent and quality in support services, assistance, and supervision to individuals, as necessary and identified in each persons Individual Service Plan on their case load and reports to the Program Director.
- Responsible to oversee support services in a quality and in a consistent manner to individuals served on their case load.
- Maintains and demonstrates a comprehensive knowledge of each persons Individual Service Plan (ISP), skill development programs, behavior support plans, medications and treatments, and general service / support guidelines and strategies.
- Monitor individuals personal grooming and hygiene needs, and assure that each person is dressed appropriately for weather and occasion. Reporting concerns as necessary to the team and Program Director.
- Encourage individuals involvement in activity planning, community integration, and socialization for people on their case load and reporting regularly to the Program Director.
Health and Safety
- Assist with health monitoring and prevention, reporting concerns to the appropriate person(s) in a timely and responsible manner.
- Reports concerns with support services and programs to Program Director, and makes recommendations and shares suggestions in a team oriented manner.
- Demonstrates and upholds universal precautions, personal protective equipment, and hazardous communication in a professional and responsible manner.
- Upholds safety for all by use of safety equipment and proper techniques of: back supports, lifts, proper body mechanics, and other safety equipment as needed/required.
- Upon delegation and training/certification - oversees medication administration per doctors orders and reports concerns / adverse reactions promptly.
- Follows current physician orders/prescriptions, current delegation standards, and applicable medical safeguards, etc.
Staffing and Supervision:
- Demonstrates professionalism and accountability through adhering to regular work schedule, attendance, and punctuality expectations.
- Records days/hours worked on their calendar accurately - as established per program/site.
- Obtains approval from Program Director for schedule changes and time off requests in advance of occurrence- as established per program/site.
Financial Accounts:
- Assist with individual funds, monies usage, receipts, signatures, recording on ledgers, in an organized and complete manner
- Minimum of College degree from accredited University or equivalent thereof and 2 years, or have at least 5 years experience in the field
- Current Drivers License with acceptable driving record is required.
- Criminal Background verification, with acceptable clearance according to applicable rules and requirements.
- Willingness to work flexible schedule and provide responsible supervision and assistance to adults with Developmental Disabilities.
- Ability to work as a team member and communicate in a positive and effective manner with persons served, support staff, and others.
- Demonstrates proficiency in verbal and written skills.
- Ability to work with diverse groups and individuals.
- Maintains current and annual training requirements.
- Must successfully complete all criminal background screens as needed.
- Must be able to safely drive a company vehicle.
- Driving history must meet insurance carrier requirements.
- Maintain current certification in CPR, First Aid, Crisis Intervention, Behavior Support Plan, and any other required training.
- Participated in personal and professional self-development to maintain or enhance work skills that allow for completion of job responsibilities.
- Works independently, performing job functions with minimal supervision necessary.
