What are the responsibilities and job description for the Peer Adherence Coach position at PALSS?
Position Title: Peer Adherence Coach
Mission: The Peer Adherence Coach will engage and support PLWH in disease management through medication adherence in a culturally conscious environment. Must also be willing to become familiar with agency services as well as local services and social support systems.
Qualifications: The Peer Adherence Coach must have a high school diploma or equivalent with a personal history of successfully accessing Ryan White Services.
Reports To: Care Coordinator Duties Performed
· Provide Peer Adherence services to newly positive clients and clients who are not newly diagnosed, but new to the agency
· Follow guidance for adherence interventions for service level consistency
· Provide program and agency information as well as community information that aides in treatment adherence
· Deliver sensitive and accurate information that encourages HIV care and medication adherence
· Motivate clients to become actively involved in their care
· Help clients develop a system of communication with providers that promote self sufficiency
· Refer cases of continued non-adherence back to case management for guidance
· Utilize good documentation skills
· Adhere to agency policies for attendance, leave, and trainings
· Ensures that Protected Health Information and other patient information is kept in confidence as required by law
· Actively participates in supervision and/or case conferences as scheduled by management as well in all agency staff meetings.
· Attends and successfully completes all required Peer Adherence Program trainings
· Performs other duties as assigned
