What are the responsibilities and job description for the Resident Care Director position at Paradigm Atlantic Management Group?
Responsibilities Include :
- Performs assessment related to the physical status and function of the resident on admission, during acute changes in condition, post-hospital or rehab discharge, quarterly, and annually.
- Uses information gathered in the assessment process to develop a care plan for each resident. Care plan must be communicated to the resident, responsible party, and assistive staff. Must identify deviation from the care plan.
- Medication management : testing of individual residents to determine ability for self-administering of medications, oversight of medication storage and administration. Communicates with certified medication aides and pharmacy to ensure medications are administered, re-ordered and filled in a timely manner.
- Responsible for ensuring staff licensure is maintained / renewed to include : CPR, CNA licensure, CMA licensure
- Development and oversight of health promotion and disease prevention programs within the community.
- Facilitates coordination with resident, responsible party, physician, and third party providers for third party services.
- Utilizes a philosophy of care that is focused on optimal physical, emotional, and spiritual well- being.
- Accountable for best care practices (for self and assistive staff); ensures care provided is in the best interest of the resident.
- On-going in service for all staff relevant to ALC license and the needs of the residents and the community.
- Ensures compliance with the Department of Community Health regulations for the Assisted Living Community license
- Development and oversight of health promotion and disease prevention programs within the community.
- Identifies common health problems in the Community and develops education programs to maximize resident well-being.
- Utilizes intra-departmental approach when coordinating health and wellness events. Confers with dietary and activities departments to create and implement unique experiences.
- Collaborates with outside health care providers to bring relevant health educational programs.
- Responsible for helping residents to engage in health promotion activities that will lead to optimal health and quality of life.
- Monitors and communicates acuity creep to Executive Director
- Supports end of life care preferences
- Responsible for developing and maintaining professional relationships with area physicians, home health agencies, and hospice agencies
- Facilitates coordination with resident, responsible party, physician, and third party providers for third party services.
