\n- Graduated from an accredited Physical Therapy (PT) college or university.
- Unencumbered Physical Therapy (PT) license for New York.
- Conduct home visit Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
- Proficient in EMR documentation (we use Hello Note but experience in this system is not required).
- CPR Certification.
