We are seeking a self-motivated Field Nurse Case Manager to cover greater Vancouver, WA and Portland, OR areas. A Field Case Manager role is a home-based position with travel, up to 2 hours one-way, to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. You must have the ability to operate autonomously with minimal oversight, and the desire to build and navigate relationships with injured workers, providers, insurance companies, employers, and attorneys.

Qualifications:
- Current, unencumbered Registered Nurse (RN) license in WA and OR
- Recommended certifications (CCM, CRRN, CDMS, CRC) preferred. REQUIRED within two years of hire date
- Valid Driver’s License, and good driving history.
