Paradigm Healthcare (PHC) provides short-term rehabilitation services and long-term care for patients in Hampshire, Franklin, Hampden, Worcester and Berkshire counties in Western Massachusetts. Our team has a decade of experience in the field and administers with a dedicated, hands-on approach that meets patients' medical needs while ensuring that patients and their families maintain the peace of mind that comes from trusting in their practitioners. We are seeking likeminded individuals who are motivated and passionate about providing quality care, alongside the skilled nursing facility team.
Essential Job Responsibilities:
- Conduct comprehensive patient assessments, in addition to standard & urgent patient care visits while updating the patients' medical record.
- Ordering, reviewing and evaluating diagnostic testing to identify the need for further treatment.
- Reviews lab, x-ray, and ED or Admission reports and initiates appropriate patient care plan.
- Prescribes medications and other forms of treatment such as physical therapy, occupational therapy to appropriately treat patients’ acute and chronic conditions and improve quality of life.
- Treat acute illnesses and diseases, as deemed appropriate.
- Treat and manage chronic illnesses.
- Reviews patient charts and ensures appropriate treatment plans are developed and followed.
- Completes all charts the same day of care.
- Educate patients and family members regarding preventive care, healthcare maintenance, health risks, etc.
- Maintains all CME/CEU, Board and Licensing requirements of state and national governing Boards.
- Perform all other related duties as assigned.
Minimum Requirements:
- Current Massachusetts NP/PA licensure and /or eligibility for certification.
- Experience in working with EMR systems
- Ability to collaborate with multi-disciplinary team, including physicians, nursing staff, social workers, and case managers.
- Demonstrate proficiency in assessment and delivery of family planning services to a diverse population.
*Salary will be based on experience and days worked.
