POSITION SUMMARY
Job Title: HFA Family Support Specialist Location: Visalia, Ca
Hours: Full-time (37.5 hours/week)

Position Description
The Healthy Families America (HFA) Family Support Specialist (FSS) will provide case management services, home visitations, assessments, intervention, and implementation of client engagement to assist in cultivating, strengthening, nurturing parent/child relationship(s), promotion of healthy child development, and enhance family functioning. The FSS will strive to help create a safe, stable, supportive family environment, which will be achieved by increasing the families’ protective factors.
The HFA FSS will be trained and adhere to all HFA model requirements and additional evidence-based practice models integrated towards programmatic goals. The FSS will be expected to conduct home visitations. Intensity of services will vary upon need as part of model requirements and complete implementation of in-home parenting curriculum. FSS will assist families from the prenatal stage to children age (5). FSS will collaborate, coordinate, and broker services that appropriately meet the client(s) needs.
Essential Functions
- Integrate all components of Case Management, which include engagement, planning, implementation, monitoring/evaluation, and termination.
- Training and implementation of HFA and additional evidence-based curriculum
- Work collaboratively with agencies and resources with the community to ensure the families meet service plans objectives
- Secure and maintain consent of all involved parties
- Maintain confidentiality of records
- Complete reports of abuse to Child Welfare Services and Adult Protective Services when necessary
- Resource and referral linkage to supportive services
- Must be able to maintain professional boundaries
- Excellent communication skills, written and verbal
- Knowledge of community resources
- Ability to work as part of a team
- Attend additional trainings as required
- Adhere to home visitation standards of practice
- Return phone calls promptly
- Ability to work with people
- Maintain positive and respectful attitude
- Ability to clearly communication with people
- Check emails daily and keep calendar up to date and accurate
- Other duties as assigned
Qualifications
- Bachelor’s Degree in Social Work or related field preferably
- Two years’ experience in case management
- Bilingual English/ Spanish is Preferred
- General computer proficiency
- Valid Driver’s License
- Reliable Transportation
- Valid Vehicle Insurance
- Pass Drug Test
- Pass Background Check
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
