Play Program Case Manager
The Play Program case manager position is responsible for providing support and resources for families that have qualified for case management. The family in conjunction with the Play Program case manager determines which areas they are in need of assistance. These goals are written down on the Family Individual Service Plan (FISP). During the course of case management goals are met and updated according to the family needs.
Case management is offered to all families with children five years old and younger. The case manger meets with the families also in their home environment. It is the goal to meet the needs of the family using resources within the community. The Play Program case manager also endeavors to develop relationships with other agencies for the purpose of assisting families in our community.
Case manager duties include:
- Computer skills in Word, Excel, and PowerPoint
- Complete Case Management tools and keep cases current
- Address client’s needs with referrals and/or Case Management
- Develop community resources to assist clients
- Typing letters for clients
- Scheduling client appointments with various social service agencies
- Case note documentation Delivering food and other donated items to assist on a case by case basis Assist family in obtaining Health Insurance
- Provide transportation to clients
- Other duties as assigned
- Minimum Qualifications:
- Bilingual Spanish preferred but not required
- BA in Social Work with 5 years of experience and/or 5 years of related experience
- Strong computer skills
- Must have a vehicle Valid CA Driver’s License
- Ability to pass a Criminal Background Check
Job Type: Full-time
Pay: $19.00 - $21.00 per hour
Expected hours: 37.5 per week
