QUALIFICATIONS:
1. Bachelor’s Degree in Human Services Field
2. Certified QIDP
3. No disqualifying events on criminal background check
4. No disqualifying events on Health Care Workers Registry
5. Pass pre-employment physical, drug and alcohol test
6. Excellent communication skills and the ability to work with participants and families who are overwhelmed with their situation
7. Strength-based thinking – able to focus on individual’s abilities and assets

1. Maintain participant case load.
2. Ensure that Individuals receive appropriate, effective, and individualized person-enter services.
3. Ensure coordination and implementation of quality services to participants within the
Home Based Waiver Program.
4. Maintain Medicaid eligibility
5. Proficient in Microsoft Word and Microsoft Excel
6. Maintain Phone and email etiquette.
7. Able to drive own vehicle to participants residence/providers/necessary meetings.

THIS POSITION REQUIRES THE EMPLOYEE TO BE ABLE TO SUCCESSFULLY PERFORM THE FOLLOWING:
Pass DSP classroom and OJT program approved by Illinois DHS
Pass QIDP classroom and OJT program approved by Illinois DHS
Understand written and oral English Language
Record bimonthly reports within participants’ files
Write legible, clear, and accurate observations in standard English
Lift 50 pounds without assistance
Stand, sit, walk, reach, grip, bend and lift as assessed by job analysis screening
Remain free of alcohol and/or illegal substance use while working; Become CPR/1st Aid certified and remain certified throughout employment
Basic Math skills.
POSITION RESPONSIBILITIES:
Identifying and gaining access to needed services such as medical, social, recreational, educational, vocational and domestic.
Coordinating services with a fiscal employer allowing for a personal support worker to be provided.
Coordinating services with the Individual Service Coordinator in supporting the outcomes and priorities outlined in the Individuals Personal Plan. Additionally, write the Annual Supports and Services Summary, and update as needed, based on assessment information and discussion among the participant, guardian, family and other members of the service planning team
Assist in applying and securing the maximum Supplemental Security Income (SSI) benefit and/or SNAP.
Understanding the criterion which determines eligibility for monthly Social Security Disability Insurance (SSDI) benefits.
Secure medical assistance, also referred to as Public Aid and Medicaid, which is required for Home Based Services Funding.
Assist with Medicaid Redeterminations and Spend Down information when requested by Department of Human Services.
Understanding the need for legal guardianship and the options in securing it.
Knowledge of community resources that may assist in obtaining a special needs trust and wills and the reasoning behind them.
Review and carryout responsibilities/procedures as identified in written/verbal communications from supervisor/administrators (i.e.; monthly reports, participant assessments, etc.)
