The Outreach Program Coordinator reports directly to the Deputy Director of Ryan White Programs and responsible for the planning, development, implementation, monitoring and evaluation of the Outreach Unit; utilizing methods, processes and techniques to achieve performance outcomes.
The Outreach Program Coordinator will work closely with the Director of Ryan White Programs to analyze partnerships and outcomes data and develop strategies for linking high risk individuals to treatment and care services.
Duties:
• Oversee the planning, development, and implementation of a strategic plan for the Outreach Unit.
• Design methods, processes, and techniques to monitor and evaluate the Unit.
• Document organizational, operational, and programmatic practices for the Outreach Unit.
• Supervise and mentor Outreach Workers and Outreach Assistant/Non-Medical Case Manager.
• Review and analyze outreach reports and deliverables received from outreach staff.
• Coordinate multi-project initiatives and deliverables to achieve program goals.
• Review outreach personnel narratives and provide a comprehensive monthly Outreach Unit report.
• Track outreach performance measures against yearly goals and objectives.
• Monitor outreach charting system through internal audits to ensure compliance with Outreach Standards of Care.
• Monitor outreach staff time of effort, unit calendar, and online data entries to ensure maximum performance.
• Attend community meetings to present services offered by Park West and the Hidden Garden program.
• Develop memoranda of understanding with community and faith-based organizations and coordinate with prevention partners for on-site HIV counseling, testing and linkage to care services.
• Maintain relationships with community partners to increase referrals of HIV positive individuals to the program.
• Oversee social media initiatives and promotional campaigns.
• Solicit and finalize content for Hidden Garden Newsletter.
• Collaborate with the Consumer Advisory Board (CAB) to assist in identifying vulnerable populations that may or may not know their HIV positive status and in need of referrals for health and support services.
• Represent agency at Planning Council and other HIV Planning Group meetings.
• Perform other duties as assigned.
Qualifications
- Bachelor’s Degree from an accredited university
- Minimum three years demonstrated experience in partnership development, achieving recruitment outcomes, and consumer engagement with vulnerable populations.
- Advanced data management and analytical skills.
