Hospice Social Worker-Full Time
The Parker Jewish Institute for Health Care and Rehabilitation is seeking a Social Worker, with a base schedule of Saturdays from 9am-5pm, to join the team in the Hospice department.

Parker Jewish Institute for Health Care and Rehabilitation is a not-for-profit adult health care system comprised of multiple health services servicing the Metropolitan New York Area.
Community and Inpatient Hospice at Parker Jewish Institute provides comfort, relief, and peace of mind to individuals with advanced or life-limiting illness and their families. We are an intimate and unique program that provides personalized care to ensure the comfort, enhance the quality of life, and preserve the dignity of our patients.
This position will include field visits in Queens and Nassau counties, in addition to in-patient visits.
Position Overview
Reporting to the Director of Patient Services, the Hospice Social Worker delivers appropriate social work services to assigned patients, residents, family members and significant others. The Social Worker is responsible for performing initial psychosocial, emotional, spiritual and bereavement assessments, ongoing assessments, and assists in the development and implementation of goal-directed IDG care plan while serving as an advocate for the patient and family members.

Position Qualifications
Master's Degree in Social Work
Active NYS Social Work License
Prior experience as a Social Worker in a community health, hospice or inpatient setting
Previous Hospice experience strongly preferred
Valid New York State driver's license and able to make field visits in Queens and Nassau counties
Fully Vaccinated against COVID-19
