Parker Jewish Institute's Community Health Services program is seeking an experienced professional to oversee the intake function for Parker's community programs-ensuring patients accepted to the program are appropriate and evaluated timely by home care and hospice staff. This is role is key in providing both quality patient care and actively working to expand our services, as Parker's Hospice and Home Care programs continue to grow.

- 1-2 years of prior as in a care coordinator, intake, admissions or community health role
- Current New York State license as a RN or LPN, preferred
- Experience in a Home Care, Hospice or Community Health environment strongly preferred
- Proficient in EMR software; familiarity with Netsmart is a plus
- Fully vaccinated against COVID-19
