Job Summary and Qualifications

Conducts individual, group and family psychoeducational groups.

Contributes to treatment plan development andattends treatment planning meetings and reviews on an ongoing basis.

Assesses environment and patients safety status and reports / responds to actual and potential safety issues or changes in the patient's condition.

Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).

Completes relevant assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.

Collaborates with the multidisciplinary team to assess and coordinate patients discharge planning needs and readiness for discharge.

Conducts post-discharge follow-up as indicated.

Evaluates re-admissions for gaps in discharge planning processes. Formulates a plan and implements new strategies.

Develops and maintains a community resource guide including access to public assistance / alternatives.

Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope.

Contributes to ensuring a safe, structured milieu with evidence-based treatment modalities / resources.

Identifies cultural considerations, gender identity and preferred language for patients and family.

Demonstrates appropriate application of age / developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.

Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers.

Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts.

Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families.

Understands state commitment processes, abuse reporting requirements, duty to warn and guardianship processes.

Demonstrates knowledge and role in hospital codes.

Assists with Process and Quality Improvement initiatives.

Performs other duties as assigned.

Practices and adheres to the Code of Conduct and Mission and Value Statement.

What qualifications you will need :

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
  • Bachelors Degree
  • Parkridge Valley offers behavioral health. We offer addictive disorder treatment services. These are for children, adolescents and adults. We have four distinctive campuses. These include Parkridge Valley Adult and Senior. These include Parkridge Valley Child and Adolescent campus. These also include Parkridge Valley Cleveland and Parkridge Valley West. Parkridge Valley West is a behavioral health hospital. We offer adult inpatient and outpatient behavioral health services. These are offered to residents of Marion and its surrounding counties.
