We are currently hiring for a Family Medicine Care Coordinator position within our Quality Improvement Department.
Responsibilities include
- Develop and implement personalized health care plans in collaboration with the Family Medicine healthcare team.
- Provide education and counseling to patients and families on managing chronic illnesses such as diabetes and hypertension, including medication adherence and lifestyle changes.
- Act as the primary contact for patients and families, ensuring continuity of care through appointment tracking, follow-ups, and communication with care teams.
- Perform retinal eye exams for diabetic patients and manage referral tracking related to care coordination services.
- Use motivational interviewing techniques to support patients in achieving self-management goals.
- Monitor and document diabetes-related data in EHR systems, leveraging tools like i2i Tracks for population management.
- Participate in quality improvement initiatives and contribute to staff training on chronic disease management.
- Bachelor's degree in Health Education or a related field, or California state licensure as a Licensed Vocational Nurse (LVN).
- Proficiency in chronic disease management and patient education.
- Strong computer skills, including Microsoft Office, EHR systems, and database management.
- Bilingual fluency in English and Spanish (written and oral).
- Excellent communication, organization, and problem-solving skills.
- Demonstrated ability to work effectively with diverse populations in a team setting.
- Sensitivity to cultural differences and a commitment to providing equitable care.
