Responsibilities
Parkview Health at Home Now Hiring LPN Home Health
Mon -Fri (8:00 AM - 5:00 PM)
- No on-call requirement with flexible schedules.
- Provides care and educates patients and their caregivers
- Coordinates services with other disciplines and community resources as required
- Evaluates and updates care plans and assessments in response to patient and environmental changes
- Attends regular case conferences, in-services, and special case orientations as required, and participates in on-call rotation

Qualifications
- Active LPN License
- One year of clinical experience as a LPN required
- Valid Driver’s License
- Proficient in technology, including smartphones, tablets, virtual reality, interactive software, and artificial intelligence
- In Washington State, all providers must meet the National Provider Identifier (NPI) requirement and provide proof before onboarding.

$24.35 - $33.47 /hour
