Home Infusion Pharm Tech / Patient Care Coord - After 6 months on site position - moves to hybrid

POSITION SUMMARY

The Home Infusion Pharmacy Technician / Patient Care Coordinator will assist and support a Home Infusion Registered pharmacist with the management of prescriptions for high dollar infusion medications, that are used for complex conditions. They will assist in scheduling clinical staff to visit patients. This may include Parkway Pharmacy Nursing and Subcontracted Nursing. Works closely with the pharmacy, nursing and production teams to determine schedule needs and availability to ensure appropriate prescriptions are being filled, delivered correct and administered accurately.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Filling prescription medications accurately and efficiently in all areas of our infusion pharmacy by utilizing the necessary technologies for each area.
• Investigate prescription benefits.
• Performing data entry of refill prescription orders accurately and assignment of directions.
• Respond to Third Party rejections and overrides.
• Refill Authorization Management.
• Responsible for reversing prescriptions, when necessary.
• Understand the various challenges of specific diseases.
• Maintain patient records and file according to company procedures.
• Update patient clinical information based on reports from prescribers and patient assessment.
• Ensures appropriate staffing is arranged for all patient care.
• Communicates with patients, to schedule infusion based on prescription refills and refers any medication or illness related questions or concerns to a board licensed healthcare professional including but not limited to: nurses, pharmacists, or supervised pharmacy interns.
• Responsible for interpreting physician’s orders and scheduling according to orders.
• Provide accurate patient scheduling inconsideration of numerous factors including knowledge of time needed for drug regimens.
• Coordinate scheduling needs with staffing availability.
• Must be adaptable to changes in workload and operations.

EDUCATION AND EXPERIENCE:

• Registered New Jersey Pharmacy Technician license required.
• Previous infusion experience preferred, background in medical or pharmacy required.
• Must possess a strong sense of urgency and attention to detail.
• Excellent communication skills both written and verbal required along with interpersonal skills.
• Demonstrated ability to prioritize multiple tasks to meet deadlines.
• Demonstrated ability to interact in a collaborative manner with other departments and teams.
• Strong critical thinking and problem-solving skills required.
• Capacity to work with limited supervision and support, in a rapidly changing and fast paced environment.
• Must have demonstrated experience to read and interpret documents such as safety rules and interpreting physicians’ orders.
• Basic level skill in Microsoft Products in order to write routine reports, create documents, enter and formatting text, and updating accurately all patient information.
• Experience with Quikbase, RX30 and Asembia 1 preferred.
• Bi-lingual a plus
