Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.
Responsibilities
The primary focus of the services of the Pediatric Complex Care Resource Centers (PCCRC) to be delivered by
the Health Care Quality Units (HCQU) will be to support caregivers in the non-medical aspects of supporting
a child with complex medical conditions receiving in-home shift care services. Many of these needs may be
family-centered and sociodynamic in nature and will lean heavily on advisory, education, and referral functions.
Families often require resources outside of what might be found among peers or extended family. The
purpose of the services of the PCCRCs are to meet the needs for information and support through focused
services to build family and caregiver capacity and efficacy.
The Family Facilitator’s role is to support families caring for children with medically complex conditions
transition from facilities/hospitals to community settings and to support the diversion of child placement in
congregate care settings. The Family Facilitator serves as liaison between departments, to provide information
to families, and assist with coordination and planning for in-home services and options for family-based
alternatives to institutions.

Outreach and Education
- Conduct outreach to families, community health related/promotional organizations, hospital/facility social workers, and discharge planners regarding in-home services and options for family-based alternatives to institutional care
- Share resources and educate families about the Office of Developmental Programs’ (ODP) registration and waiver processes
- Provide technical assistance to stakeholders
- Work closely with Health Care Systems, Managed Care Organizations (MCO), Department of Human Services (DHS) offices to accept referrals of children in need of permanency with a family or family like or community-based setting
- Work in conjunction with state entities to support planning for children and policy planning efforts
- Work in collaboration with Support Coordinators and Special Needs Unit case managers from managed Care Organizations (MCO) to ensure safe discharge and transition planning by supporting the coordination and setup of home health services and assuring that any accessibility issues are addressed
- Establish and maintain relationships with social workers or discharge planning staff in all pediatric hospitals and pediatric congregate facilities in covered geographic area to ensure timely communications about admissions and discharges
- Support/encourage planning efforts for safe discharge and transition home that include relevant parties
- Assist family in working with local school district to establish any educational supports or resources
- Act as a transitional coordinator until the child has permanency
- Maintain periodic follow ups, as needed
- Attend meetings with other Family Facilitators and representatives from the Department of Human Services (DHS) to provide updates, identify barriers for families and discuss strategies

Qualifications
- Bachelor’s degree in one of the following: social work, learning and instruction, human services or related field
- Life experience preferred in working with Managed Care Organization (MCO), Supports Coordination, Administrative Entities and/or medical service providers
- Complex medical knowledge and mental health knowledge preferred
- Intellectual Disabilities systems knowledge preferred
- Act 33 and Act 34 clearance and FBI background check, if applicable
- Valid PA Driver's License
