Job Title: Project Coordinator, Community Wellness
Salary range: $19.00-$20.54 per hour
This is a hybrid position, where the staff must be willing to commute to our San Fernando office and travel throughout the greater Los Angeles area as part of the role.

As the Project Coordinator for Partners in Care Foundation’s Community Wellness programs, you’ll play a vital role in shaping and expanding health promotion initiatives that support and empower older adults and adults with disabilities across the city and county of Los Angeles. You’ll work directly with community partners, organize impactful workshops, and help spread awareness of our life-changing programs.

What You’ll Do:
- Build & Cultivate Relationships: Forge meaningful connections with community organizations throughout LA to introduce and embed evidence-based health programs.
- Coordinate & Execute: Work closely with leadership and fellow team members to schedule workshops, recruit participants, and keep everything running smoothly with your strong organizational skills.
- Engage & Empower: Lead workshops in diverse settings, including senior housing, community centers, faith-based organizations, and more—helping participants gain valuable knowledge to enhance their well-being.
- Data Management & Reporting: Keep accurate, up-to-date records and ensure data confidentiality, safety, and quality assurance in line with HIPAA standards.
- Customer-Centric Approach: Serve as a friendly and resourceful point of contact for inquiries, assisting with program referrals and workshop enrollments while delivering exceptional service.
Required:
- Valid California driver’s license, reliable transportation, and current liability insurance.
- Bachelor’s degree from an accredited university in applicable fields (e.g., public health, health education, social work, and gerontology, etc.).
- Experience with Microsoft Office Suite (Word, Outlook, Teams, and Excel) preferred.

- This role involves frequent travel across the greater Los Angeles area to visit community partners, facilitate workshops, and engage with program participants. A reliable vehicle and the ability to travel independently are essential.
